Welcome to the registration for the Dementia Friendly Newtown Champion Training. In order to participate, you must have previous attended an in-person or virtual 60 minute Dementia Friends Information Session.
Please complete the registration below. Make sure to provide a phone number and email address. The form will ask for the name of the Dementia Friends Champion who provided your Dementia Friends Information Session. Please enter - Anneliese and Stacie if you plan to attend the Dementia Friendly Newtown Dementia Friends Information Session on July 23, 2020. Otherwise, please enter the name of the Champion who provided your session.
Once we receive your registration, you will receive a separate email with information on how to join the session via ZOOM
